Larry, the cam has two flat spots on it with one end being bigger than the other. The big part of cam goes toward the rear axle. Then the cam levers go up and toward the rear. Knowing this you should be able to figure out which side is which and then put them on. The hole in cam should line up with hole in the lever when all is OK. HTH.

Sounds like you're trying to put the cam in the wrong hole. "Up or Down?" The pointed end faces forward (toward the drum). The cam and bushing go in the hole between the radius rod bolts.

Yes, you're correct about left and right cams. The levers will fit either side. With the brake lever up and angled back slightly, the cam should align with the holes in the lever. (Pointed end out.)

Larry, when you first insert the cam, place the brake shoes in place, and manually turn the cam. Horizontal, the brake shoes are closer together, away from the drum. When the cam is vertical, it expands the brake shoes against the drum.
