It is shaped like a pulley. Maybe for pulling the car out of mud?
The radiator looks pushed back a bit.

Zoomed in on the photo, that pulley shaped thing is an accessory tail or perhaps tail/stop lamp. You can see the wire and socket plug hanging out the rear. Haven't seen that style before, seems made to mount to the top rest arm, pretty cool.

Other accessories are locking steering wheel, strap to hold the crank handle up, accessory safety dual wishbone, (owner has kids and wants a safe to handle T on rough roads) , a dog bone Motometer cap, and tool box on the running board. :-)

I don't think the radiator is slanted.

If you take a photograph of a photo and the photo is not perfectly flat and/or the camera lens is not perfectly parallel to the photo, then you will get some distortion. This can be avoided by scanning the photo on a flat bed scanner.
