Herb,
Thanks for posting. I cannot tell if that is reflection on the black paint or brass trim on the headlights and sidelights. If brass then 1915. If black paint then late 1915 to 1916.
Smith Form-a-Truck was one of the more popular conversions and looking at some of the photos at:

But other companies also offered chain drive conversions.

They look black next to the radiator, and also the sidelights look to have black rims and tops... so I'll venture to say it was a '16. And man, I sure do like that truck body.
