Does anyone have parts for one of these? One of the Jack screws is missing two teeth.

If the problem is in the screw itself, you may be able to obtain acme threaded rod from McMaster-Carr. If it's some other thread or an odd size, a machinist with a lathe should be able to reproduce it. That's a nice item.

One of the bevel gears that the screw passes through is missing two teeth.

A good welder could rebuild the teeth, then they'd have to be hand-shaped or machined.

Ed, an old time repair is to drill and tap for a couple good size bolts, maybe 1/4-20 or maybe even 5/16. Do a couple side by side where the tooth is missing, and get them as deep as you can. After tightening the bolts, cut them off and grind away the sides a bit. Now braze the tooth profile, and file to shape where it works like a tooth. You want it to turn smooth, with no slop.
The gear is cast iron, and I don’t think any type of weld will hold up.
I bought a “perfect” weaver from a guy through the mail a couple years ago, and that’s how I had to fix it. JMHO for what it’s worth.
