What Keeps the Lock Nut In Place?

Here's a simple question, but one that has me scratching my head.

Regarding the lock nut that goes on the end of the camshaft...the one that keeps all the timer guts in place...what keeps it in place if there's no cotter pin? It seems like the rotation of the cam would loosen it over time.

Since the cam is rotating CCW as you look at it, the nut goes on CW, stays tight just by natural motion.
