As an answer to many questions - yes, it IS for Model T s. The two bolt holes are same as T head water outlet bolt holes so pump pulley is perfectly above fan pulley. So a longer fan belt will be required to run all 3 pullies. The impeller is located where the hotter coolant gets ‘pushed’ to the upper radiator tank instead of the pump monstrosity guys used to install on the left side.

Its not unusual, or rare - I've seen many of them - have two in my collection of parts, one brass, one iron. Since made in Stockton, maybe more exist in California.
