Bolt Chart Update - Help Requested

I prepared an update of the bolt chart over a year ago now. This is available in the 'Gallery' section.

However, despite my best attempt there were a few errors lurking in the chart and more than few missing bits of data.

However, several forum members sent me details of the corrections and some extra information needed and I duly set forth to update the update.

However, time moved on and, yes you guessed it, I have misplaced those details kindly sent to me.

However, I am hoping that you may remember your contribution and perhaps post the corrections and new data below so that I can (at last) get around to fixing those pesky errors still hiding in the detail.

Thanks Steve, yes this is the original chart I based the update on. The update (in the gallery) uses this and has extra information added where I could get it from, still leaving some 'yellow cell gaps' and some errors to correct.
The original was put together back in '09 by Joe Boyers and Robert Ducharme.
