Does anybody know who makes the pedal extensions for the clutch and brake pedals?

I do not know, but Tom Strickling makes the reverse pedal extension, so he may make the other extensions too.

I don't believe I've ever seen a newly manufactured clutch/brake pedal extension. Just old ones from the era.

If you don't mind the non-original look, you can make your own with 2" square tubing. Fits any pedal.
The inevitable often happens.
